Baltimore CONNECT is a 501(c)(3) non-profit organization working to better link social and healthcare services across Baltimore. CONNECT was formerly a Patient-Centered Outcomes Research Institute (PCORI)-funded community engagement study between 20 community-based organizations and the Johns Hopkins Health System.
