Our mission is to create and strengthen a collaborative network of community, faith-based, and neighborhood organizations that advocate for the needs of people in Baltimore, and CONNECT them to comprehensive and coordinated health and human services.

We envision a future in which health care entities, social service organizations, and people in the community work together seamlessly to achieve optimum health and well-being for all.


Partnering With the Community

Baltimore CONNECT’s mission is to create and strengthen a collaborative network of community, faith-based and neighborhood organizations that advocate for the needs of people in Baltimore, and connect them to comprehensive and coordinated health and human services.

We comprise a network of over 40 community-based organizations (CBOs) that predominately serve low-income individuals and families in East Baltimore.

Given the breadth and scope of their individual capabilities, each Baltimore CONNECT CBO is able to individually serve hundreds of families each week. The diversity in skillsets of our CBOs enables us to serve many different populations. Many of the residents served by CONNECT CBOs seek assistance navigating health insurance matters or the healthcare system broadly. In some neighborhoods, our CBOs serve residents who are uninsured or underinsured. While facilitating connections to health insurance and healthcare, our CBOs prioritize the social needs of the residents in order to ensure they can make their health a priority.

Strategic Priorities (2021)

  • Nurture and expand our network of CBOs in Baltimore

  • Focus intently on improving the health of residents of Baltimore City (health equity)

  • Create public-private partnerships with Baltimore-based organizations and non-profits to expand grant opportunities for CBOs seeking to address health equity

  • Collaborate with Johns Hopkins Health System (delivery arm) to achieve outcomes related to total cost of care (TCOC) goals
