Roots of Wellness Program Launches to Support Community Health in Baltimore

Baltimore CONNECT, in collaboration with the Johns Hopkins School of Nursing and other local partners, is proud to launch the Roots of Wellness program — a free health and wellness support program for Black and African American residents in Baltimore and nearby communities.

What Is the Roots of Wellness Program?

Roots of Wellness is a community-based initiative designed to support residents who want to take control of their health in a supportive, culturally responsive environment. The program offers:

  • 1-on-1 health coaching with trained Community Health Workers (CHWs)

  • Referrals to trusted local services, including food, housing, and medical care

  • Group wellness sessions that foster connection, healing, and self-care

  • Up to $175 in gift card incentives for active participation

This program is completely free and centers the needs of residents most impacted by chronic conditions such as:

  • Diabetes and pre-diabetes

  • High blood pressure

  • Overweight and obesity

Who Is Eligible?

You may qualify if you meet the following criteria:

  • Identify as Black or African American

  • Are 18 years or older

  • Live in Baltimore City or surrounding areas

  • Have diabetes, pre-diabetes, high blood pressure, and/or are overweight

Why This Matters

Health disparities continue to impact Black communities across Baltimore. Roots of Wellness was created to change that — by meeting people where they are, building trust, and offering personalized support rooted in lived experience.

By collaborating with the Johns Hopkins School of Nursing, local health systems, and community-based organizations, we are working together to create health equity in Baltimore.
