Roots of Wellness is an 18-month community-based health program designed to improve health outcomes for Baltimore residents through the trusted support of Community Health Workers (CHWs).
Our mission is to reduce health disparities by providing free, accessible resources that address chronic conditions and strengthen community connections.
What Participants Receive
Free 1-on-1 health coaching with a trained Community Health Worker
Comprehensive health screenings to monitor key risk factors such as pre-diabetes, diabetes, high blood pressure, overweight, and obesity
Connections to local resources including nutrition programs, fitness opportunities, and healthcare providers
Group wellness activities focused on building resilience and community support
Up to $175 for full participation over the study period
Who Can Join
We are currently seeking individuals who:
Identify as Black or African American
Are 18 years or older
Live in Baltimore City or surrounding areas
Are living with one or more chronic health conditions: pre-diabetes, diabetes, high blood pressure, overweight, or obesity

In October 2025, we will schedule enrollment appointments, with in-person enrollment during the first two weeks of November 2025, while health data collection will begin at that time.
Our Progress
So far, we have screened approximately 100 Baltimore residents and are continuing outreach at local community events over the next month. Our CHWs are on the ground, connecting directly with residents and ensuring that health equity is more than just a goal—it’s a reality.

Baltimore communities face higher rates of chronic health conditions and barriers to care. By using Community Health Workers who understand local culture and challenges, Roots of Wellness builds trust, access, and lasting change. This program is a vital step toward a healthier, more equitable Baltimore.
