Baltimore CONNECT, in collaboration with Young Successful Leaders, is seeking a dedicated and compassionate Community Health Worker (CHW) to join our team full-time. This is an exciting opportunity to be part of a growing initiative that focuses on improving the health and well-being of Baltimore residents, especially those managing chronic conditions such as hypertension, diabetes, and obesity.
As a CHW, you will support individuals through personalized health coaching, community outreach, and wellness workshops, using both in-person engagement and digital health tools to promote sustainable health outcomes.
Key Responsibilities
● Plan and implement outreach activities for participant recruitment and sustained community engagement.
● Conduct 1-on-1 health coaching sessions focused on chronic disease management, emotional wellness, nutrition, exercise, and other lifestyle factors.
● Help participants set achievable health goals and co-create personalized action plans to support long-term wellness.
● Facilitate group wellness workshops, support groups, and educational sessions virtually, in-person, and through social media platforms.
● Use digital tools like The Medi Platform to deliver resources and connect participants to local health and social services.
● Provide general support to participants by assisting them with navigating online platforms, building digital literacy skills, and accessing health services.
● Collaborate with healthcare providers, community-based organizations, and social service agencies to ensure care coordination.
● Accurately document all participant interactions, referrals, and outcomes using data collection tools in compliance with program guidelines.
● Maintain confidentiality and uphold the highest ethical standards in all participant interactions, including full adherence to HIPAA regulations and data privacy policies.
● Stay up to date with emerging trends and best practices in community health, chronic disease prevention/management, and health equity.
● Support occasional community events or initiatives led by Young Successful Leaders that align with CEAL-DMV programming.
Qualifications
● High school diploma or equivalent required.
● Previous experience as a Community Health Worker, health coach, or similar role.
● Maryland CHW certification required, or willingness to obtain within 60 days of hire.
● Experience facilitating health education or wellness-related group sessions.
● Strong communication and interpersonal skills, with the ability to connect effectively with diverse populations.
● Familiarity with chronic disease prevention, nutrition, emotional wellness, and holistic health approaches (training provided).
● Ability to work independently and collaboratively in a team-oriented environment.
● Comfort working with digital tools, social media platforms, and virtual communication technologies.
● Proficiency in electronic data collection and reporting.
● Familiarity with Baltimore City neighborhoods and community resources is preferred.
● Reliable transportation is required.
Additional Information
● Salary: $40,000-42,000 annually
● Schedule: 37.5 hours per week, Monday through Friday, with some evening and weekend hours as needed.
● Duration: This is a grant-funded position renewed annually based on funding availability and performance.
Accountability and Reporting
● Must attend all required team meetings, training sessions, and weekly supervision check-ins.
● Provide weekly work schedules and consistent updates on participant engagement, recruitment, and progress.
● Complete required documentation accurately and on time, including CEAL-DMV grant deliverables and organizational reporting.
● Demonstrate ongoing commitment to professional development, ethical practice, and compliance with organizational policies.
● Performance will be reviewed each year through a collaborative evaluation process by Young Successful Leaders and the Baltimore CONNECT CHW Supervisor.
