Young Successful Leaders (YSL), a re-grantee of Baltimore CONNECT, is seeking a full-time Community Health Worker (CHW) to support the CEAL-DMV initiative, in collaboration with Baltimore CONNECT and Johns Hopkins. This grant-funded role focuses on community outreach, 1-on-1 health coaching, and group education to support the prevention and management of chronic conditions such as high blood pressure, diabetes, and obesity.

The CHW will work directly with community members to promote health equity, improve access to care, strengthen digital and health literacy, and connect participants to local health and social services. This position is ideal for individuals passionate about community-centered care, chronic disease prevention, and addressing social determinants of health.

Location: Baltimore City
Schedule: Full-time (37.5 hours/week; some evenings/weekends)
Duration: Grant-funded, renewed annually based on performance and funding
